We are centrally located in Ballarat and have free onsite parking for your convenience.

Before your first appointment, download and complete the medical history form below. You can hand this to our receptionist upon your arrival or send it to us in an email.
Health Insurance
We accept ALL private health funds through our HICAPS terminal, all you have to do is ‘pay the gap’.
We also honour the Child Dental Benefits Schedule (CDBS) for all eligible children aged 2-17 years of age.

Payment Options
At Dental On Errard, we specialise in providing affordable dental treatment for the whole family.
We accept EFTPOS, all major credit cards, and all health funds through our HICAPS terminal. Please remember to bring your insurance card with you to your dental appointment, so we can claim your rebate for you on the spot.
Financial assistance can be provided by DentiCare, Afterpay, Zippay and by accessing your Superannuation. Ask the reception staff about our payment options and access the treatment you and your loved ones need as soon as possible.
Dental On Errard requires at least 24 hours’ notice to cancel an appointment. A fee of $50 may be applied if you fail to attend your appointment or cancel within the 24 hour period before the appointment.
Failure to respond confirmation texts or phone calls sent by Dental On Errard may result in an automatic cancellation of the appointment. Confirmation is required for your dental appointment to go ahead.
Payment is required on the day of treatment.
